I've found no magic answer, but several little things that each help some. Get good bicycle gloves with gel padding. Use good, cork, grip tape, wrapped nice and thick. Change grip positions often. Remove one hand at a time and sling it around a bit to keep good blood flow going. Do not lock your elbows. Locking your elbows puts all the vibration directly into your hands and wrists. Sit up a little straighter, either by moving the seat forward a little, or by raising the stem a little. Leaning way over places more body weight directly on the hands.
